Overview

Training designed to educate pharmacists on how to best implement interprofessional collaboration within clinics and with local providers to better care for patients with chronic conditions. This year, focus will be on many of the "soft" skills that are needed to effectively work with chronic conditions, with a few clinical session thrown in as well. Learners may take one or all of these courses but completing the full series (8 classes for 8 hours of CE) will earn them the 2024 Clinical Pharmacy Pathway Certificate from Kansas Pharmacists Association. Choose the date/time option for each module that best fits your schedule - each course will be offered once as a live webinar, then available on demand as well.

Course Summaries


  • April 17th, 1-2pm - Positively Impact Communities by Understanding Social Determinants of Health - Dr Cassie Shields
  • Social determinants of health (SDOH)- the conditions in which people live, learn, work and play- have become challenging obstacles in delivering pharmacy services, especially in rural areas.  By identifying SDOH, pharmacists and staff can improve patient's health through a multitude of opportunities including medication adherence, collaboration and access to other resources in their communities.
  • April 18th, 1-2pm - Trauma-Informed Pharmacy Care - Vanessa Lohf
  • The experience of trauma and toxic stress is widespread in our communities, having a lasting impact on physical and mental wellbeing. Yet relatively few in the healthcare field are familiar with those impacts and how trauma-informed care can transform clinical practice – for both staff and patients. In this session, pharmacists will learn more about the science of Positive and Adverse Childhood Experiences (PACEs), their affect on long-term health, and how to apply this information to daily practice to improve chronic disease management outcomes while also promoting provider wellbeing.
  • April 19th, 12-1pm - Easing Aching Joints: Treatment of Osteoarthritis - Dr Chelsea Love
  • Osteoarthritis is the most common form of arthritis and affects a significant portion of the population on a daily basis. Pharmacists can have a firsthand role in the treatment of osteoarthritis as many treatment options include non-pharmacologic therapies and over the counter medications. The most recent guidelines were updated in 2019 and continued research has shown the effect of osteoarthritis on other chronic diseases. This presentation aims to provide a summary of osteoarthritis treatment guidelines and the importance of managing osteoarthritis symptoms.
  • April 20th, 9-10am - 2024 ADA Standards of Care Updates - Dr Madison Auten
  • This presentation is meant to enhance your knowledge of the ADA Standards of Care as well as inform clinicians on diabetes management along with comorbidities often seen in patients with diabetes. The goal of this presentation is to challenge clinical practice of diabetes management to align with guideline-directed care.
  • April 23rd, 1-2pm - Enhancing Interprofessional Communication: Tools for the Pharmacist - Dr Abby Petrulis
  • This presentation aims to provide pharmacists with tools to enhance communication between professions. Pharmacists are highly educated and able to discuss clinical problems and medication recommendations on a high level, however the acceptance rate of these recommendations can depend on the delivery of the messaging. After completing this program, pharmacists will have an enhanced toolkit with which to discuss medical problems and interventions with a patient's care team.
  • April 26th, 12-1pm - Beyond Diet: Advances in Obesity Management - Dr Mariah Black
  • Obesity is an epidemic in the United States, and is associated with significant morbidity and morality. Over the last decade many medications have been approved for chronic treatment of obesity, and in the last 2 years has prescriptions for these medications have dramatically increased. This presentation aims to provide relevant background on obesity pathophysiology, consequences of obesity, and treatment options for obesity.
  • April 30th, 12-1pm - Therapeutic Lifestyle Changes - Dr Ashley Crowl
  • This program will review lifestyle recommendations and several different dietary approaches recommended in the management of chronic conditions, such as, Hypertension, Hyperlipidemia, and Diabetes Mellitus. Goals include being able to distinguish between different evidence-based meal plans, identify physical activity goals, and consider other lifestyle changes that lead to overall improved health.
  • May 1st, 1-2pm - Finding What Matters: Intro to Motivational Interviewing - Dr Amanda Applegate
  • This presentation is meant to provide pharmacists with an overview of the tools used in motivational interviewing (MI). By working with patients to identify their personal motivations instead of directing them to change, pharmacists can encourage meaningful lifestyle changes to improve self-management of chronic diseases.

    The Kentucky Education Research Foundation is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. This series is approved for a total of 8 contact hours of continuing pharmacy education (CPE) credit, or 1 hours per module.
